In in\ncollaboration with the CHA, Southminster Inc., and other community\norganizations propose a comprehensive approach to assess the needs of\nresidents, provide adequate health and social services, and identify best\npractices to promote successful aging and quality of life for older adults.\nThis proposed <strong>Aging in Place Program<\/strong>\nfor residents 55 year and older in affordable housing expands the vision of the\nCHA to address public housing in Charlotte by using a holistic community redevelopment\napproach, and focusing on multiple critical factors, including mixed-income\nhousing, educational opportunities, youth and senior programs, health and\nwellness services. <\/p>\n\n\n\n<p>The proposed <strong>Aging in Place Program<\/strong> will have the following <strong>AIMS<\/strong>: <\/p>\n\n\n\n<ol class=\"wp-block-list\"><li><strong>Assess<\/strong> the health needs and supportive medical and social services for resident seniors at affordable housing<\/li><li><strong>Coordinate<\/strong> medical and social services to improve sustainable aging in place and wellness among senior residents<\/li><li><strong>Evaluate<\/strong> the impact and determinants of the \u201cAging in Place\u201d program to promote quality of life for senior residents at affordable housing.\u00a0 <\/li><li><strong>Promote <\/strong>leadership development among senior residents in affordable housing.<\/li><\/ol>\n\n\n\n<p>The\ncurrent proposal follows the holistic perspective on successful and healthy\naging.&nbsp; It will examine the conditions\nunder which residents at affordable housing would maintain functional capacity\nand productive lives as they experience changes in multiple domains.
